All fighters need blood work before they fight, Hep B, Hep C and Hiv.  Must be negative results on all three tests.  Blood work must be done once a year, call 1-888-PEACE4U to request a test for a 2 day turn around for all three blood tests.  The test costs $109.00. Request the test results be faxed to Samurai Fighting Arts Academy , fax # 541-773-4484 and also fax results to the Boxing Commission.  Then call us at the dojo and make sure the fax was received, the Dojo phone number is 541-773-2990.

All fighters are being issued federal ID, stating they are registered cage fighters in the state of Oregon.  If you do not have your ID you must show up at weigh ins and bring valid photo ID and fill out federal ID form, before you fight.  Federal ID card with your picture on it will then be made and sent to you in the mail, stating you are a register cage fighter in the state of Oregon.
